The practice would like the question asked on the form to map to all sections of the health summary, not just the "general" section

It would be helpful if there was a way to schedule forms to send at a later date. For example, when I schedule the appointment I'd like certain screening forms to go to the patient only a few days in advance so they don't fill them out too soon. Right now I have to set a reminder for myself to send them.

Currently, you have to download any attachments from forms that patients send in and upload to the patient chart. Cut that process out, by having the upload automatically go into the patient chart.

Add a notes section to forms so the provider can document after reviewing the form

Ability to have the DOB populate into forms - the issue might be with the DOB living on the Core side

There are profile mapping options to add patient information into a form automatically. Currently, it only works for certain patient demographics (name, email), but does not work for all the other options on the profile mapping dropdown options.

It would be nice to have the full ability that IntakeQ offers. Other than consent forms and questionnaires, they offer Progress Notes. I think that would be so much better than the notes set up in clinical.

It would be extremely helpful if we could pull up the forms in the interaction screen from the patient chart and have them fill it out in the office. Having to send them the form or pull up a link and then assign it to a chart confuses people easily.
