We have had some feedback from patients in regards to the membership enrollment forms. Specifically, on page 2 (Additional Detail for Primary Member". At the bottom of the page, the title "Enrollment Status" appears to be included within the section of "Healthcare Insurance". Because of the location of the membership enroll me/do not enroll me location, many people have been confused. Would there be a way to change either the location of this question and/or the layout so it does not appear to be tied to the Insurance section of enrollment?
