When we send out a fax and select to attach it to a patient chart, the patient name (and DOB) should also auto-fill onto the letterhead of the fax, as patient identifiers. We assumed this was happening. Otherwise the recipient does not know who we are faxing about unless we hand type all of that into the comments.
