Make 'No active allergies' active selection/endorsement

For patient safety and medicolegal purposes, providers need to actively endorse or confirm selection for 'No active allergies' (better yet: 'No reported allergies') with date last updated, rather than that being the default selection if no medication allergies are listed - this helps both primary and collaborating providers to know if 'No allergies' is actually current/correct or first needs to be asked before prescribing, etc.
