The sequence for appointment scheduling and online signups should be all in one. Patient or non-member should be able to go to first book an appointment date -> which then requires completion of payment/signup if applicable for a charge (which should be possible to designate as $0, not possible right now) -> prompts to complete intake forms/submit documents if the appointment requires (intake forms are per visit type and member vs non-member, only the appointment, signup and payment should be actually required to complete the booking). If a patient is a member and their plan allows a certain number of a special type of visit free per year, then the payment should take that into account when recognizing the patient's plan and charge accordingly.
