Lack of date and other relevant information when exporting a visit note or full patient chart export.

When downloading a patient visit (or other interaction) to a PDF, for example to include with a referral, or when exporting the full patient chart, the generated document does not contain any dates for the interactions/notes, it does not say which clinician signed the note, and the formatting is abnormal/unprofessional-appearing. It is extremely cumbersome to have to go in and add in the visit/interaction date and clinician, but you can't send prior records to another clinician without any dates or names on any of the clinic visits or other interactions! And it's very frustrating to have to copy and paste the text of the note into my own Word document and then save that, since the exported PDFs come out with such weird formatting.

The header of clinical notes really needs to have the patient information and date. Currently it's just the clinic name and provider name. Then when printing, making a PDF, attaching to a referral or sending medical records, these clinical notes have no patient info and no date on them. This seems a huge medicolegal issue just waiting to happen.
We really need the patient name, DOB, and other identifiers along with the date of service to be the first and most prominent thing on clinical notes.

We are new to Hint and this has been one of the most frustrating things we have come across. It really makes no sense at all. We would love to have this corrected. I can't imagine there are not many more who have concerns with this process.
