Most specifically this relates to pediatric practices. Charge one fee per membership, rather then for every member. Per primary does not work for pediatric practices, because the majority of the owners are not on the membership themselves, but rather just payers. Practice does not want commission added for every patient on the membership, but rather just once for the whole membership.
