Exporting and tracking for preventative care.

I'd like to be able to export by preventative care. I'm currently putting these in health history or vaccines. But I'd like it to have it's OWN category. AND be able to track it, it should put their age at the top of the list.
Things needed would be (grade a and b according to the USPSTF)
-Cervical Cancer Screening (age 21-65),
-CRC (age 45-75), Folic acid (childbearing age),
-Hep B (pregnancy),
_HIV screening age 15-65,
-HTN screening (this should auto populate from most recent vitals taken.),
-Neonatal ocular GC prophylaxis,
-Syphilis screening,
-Tobacco cessagion,
-AAA, MEN age 65-75
-Anxiety disorder aged 8-64
-ASA prevention
-Asx bacteruria (pregnant women)
-BRCA related cancer screening in women with personal or FH
-GC/C
-Depression screening (12+)
-Falls/prevention (65+)
-Obesity/
-IPV
-Lung CA (adults 50-80rs old with 20py hx or have quit within last 15 yers)
-osteoporosis
-DM/Pre DM
-STIs
-Skin CA prevention
-Statin use for primary prevention
-Unhealthy alcohol use
-Vision in 6mos to 5yrs old
-OTHER (I'd like to be able to track my own things here)
They would need the DATE performed/done, and DATE next due.
