I am having a problem with a patient that swears that she didn't sign a contract when she signed up online. Now she wants to cancel her membership. She wants a copy of the contract she signed printed out and given to her to prove 1. that she signed a contract, and 2. that she owes the cancellation fee listed in said contract! This would be extremely helpful. Thank you.
