Automated intake form delivery that is automatically linked to the patient chart.
Description:
When a new patient becomes active (e.g., after paying for a membership), automatically send an intake form in a way that it is assigned to the patient chart so the completed form returns to that patient’s chart and can be mapped into their medical record.
Current limitation:
Today, only sending a form from the patient’s chart creates that chart link; public URLs can be shared, but they won’t map into the chart unless assigned to the patient chart. There are, to our knowledge, no integrations/partners that would enable or allow for this.
Use case:
Trigger on “new patient becomes active” and send the form (or link to Hint-maintained form) via email or SMS from the patient chart.

This may already be achievable, in which case I need help!
We have a medical history intake form that we want all new patients to
complete. It would be great if the patient could complete that form upon
online sign-up. Right now, patients can sign agreements (e.g., membership
agreements, good faith estimates, privacy notice, telemedicine consent, etc.),
but not complete any forms (as far as I know).
One lukewarm workaround is to redirect the patient to the form’s public URL
after they’ve signed up. However, I still need to go into Hint and assign that
patient’s form submission to their chart.
My current workflow is to “send form to client” each time we get a new patient
sign-up notification, but there are opportunities for error (e.g., I forget, I
send the wrong form, I send it to the wrong person, etc.). Automation here
would be nice!

Additionally the information goes from the form TO the correct hint section. Like when they fill out meds, it goes to the meds section. When they fill out insurance, it goes to the insurance section.
