Allow for employees and dependents to be on different plans

Would like the ability to add employees and dependents to different membership plans. If a patient is engaged they are charged a different rate and this is done on a patient by patient basis. In the current state if the plan is updated it is updated for the entire family. To allow for the plan to be updated for each individual all dependents have to be listed as an employee which makes it difficult to track employee dependent relationships.

First Primary Care offers a full health plan to employees. If the family does not choose for the dependents to enroll in the full health plan (ie, they have their insurance through their own job) then they are set up on a DPC only plan. This means the employee and dependent are on different plans. In order to track this appropriately they also have to enroll everyone as an employee so they can assign them the correct plan.
