When a form is returned to a practice's forms section, this feature request would allow a "assign to patient" button so you don't have to download it and then upload to the patient's chart.

When patient fill out a form from a URL it just loads into the Forms Submission queue and doesn't link to their record. At a minimum, there should be a way to click the ellipsis on the far right and "send to chart" by selecting which patient chart it belongs to. Otherwise the form has to be printed to PDF and then manually uploaded. What a waste of time.

When sending an IntakeQ form URL link in an email for patients to fill out, if they use email or phone to login to form then it should automatically sync to their chart rather than showing up as an anonymous form in the Form Submission queue.

To map a form to an unregistered member, go to settings > communications > forms. Select the form that has been submitted, and assign it to the member of your choosing.
