Add column for patients' current provider to the “Advanced: Network Patient Employers” report

It is important for our affiliates to see what each member's provider and employer is for a couple of reasons, but here is the main reason for context:
For members that are on engaged plans, we need to know the date of the first appointment. Our affiliates gather that information for us and reports that back.
Example: if the report shows the affiliate they have 10 Senox members, they will need to look up each member to see what provider they are assigned to, then reach out for appointment details.
If the report showed the provider name, this would save them from looking up each person.
